As we welcome Alzheimer patients into our North Fork home, our staff delivers high-quality care in a safe, clean, warm and nurturing environment so they can provide ultimate comfort. Our interdisciplinary team ensures that residents with Alzheimer’s disease have access to the most appropriate, high-quality care that they and their families expect and deserve.

As part of our patient-centered approach, we strive to educate the family to meet any advanced needs that arise. For the benefit of the patient, we encourage family and friends to participate in care while offering unrestricted visiting hours.

Our thorough planning includes discussion on discharge policy, the availability of continuing care as needs change, and hospice and comfort care.
